I, Rajesh Singh Sori, residing at Central Jail Line, Raipur, with permanent address in Udaipur, District Surguja, Chhattisgarh, came to Saubhagya Hospital for my wife's treatment (for pain during her periods) and met Dr. Shalini ma'am.
